Once you are ready to make your selections, please complete the following:

• Complete this Enrollment <form> and return to Jacob Elliott.

 SIMPLE IRA Retirement Plan: 

• If you are already enrolled and would like to change your salary deferral, complete the Salary Deferral Form and return to Jacob Elliott.
• To enroll into a SIMPLE IRA account, please complete the SIMPLE IRA Enrollment Packet and return to Jacob Elliott.   

Navigate to the Blue KC website for Medical, Dental and Vision Information. 

Log In if you already have an account to obtain more details about the following topics:

If you don't have an account with Blue KC and are considering a plan, please review the following options:

To search for a Dental Provider, please select Find a Dentist and choose one of the following options: 

• Search for Dentists Nationwide
• View & Print Dental Provider Directories
• Visit the Dental Resource Center and learn more about Dental Coverage

To search for a Vision Provider, please select Find a Vision Provider and choose one of the following options: 

• Search for Eyemed Vision Providers Nationwide
• View & Print Blue Vue (INSIGHT NETWORK) Vision Provider Directory

No Vision ID card is required, provide your Social Security Number during your office visit.

• Complete the Enrollment Form  within the first 2 weeks of employment and return to Jacob Elliott.  

Simple IRA Retirement Plan: 

• New employees are eligible to participate on January 1.
